If you haven't submitted shared prize forms for championship round prizes they are available and the deadline to submit them is Monday Jan. 3.

Please email me to receive a form ([email protected]). The forms must be notarized and emailed back to me by that day. If you have already submitted a form you do not need to submit a new form. If you have any questions let me know.
