Shared Prize Forms Due Dec. 12

Shared prize forms are available for regular season prizes in the NFFC. If you wish to use one for your prizes the deadline to submit it is below:
Monday Dec. 12
Please email me to receive a form ([email protected]). The forms must be notarized and emailed back to me by that day. NOTE: If you submit a form it will be used for ALL prizes you win in those leagues in both the regular season and championship round UNLESS you contact me and say otherwise.
