W9 Forms Needed For 2009 NFFC Prize Winners

Everyone, we're nearing the end of Week 13 and that means our first round of NFFC prize winners will be announced. In order to receive a check for any winnings from us, we need to have a W9 form filled out from you on file. If you've won money in the past and none of your personal information (such as your mailing address) has changed, you're all set. But if you will be receiving a prize check from us for the first time or if any of your personal information has changed we will need the W9 form filled out and sent back to us.

If you are in position to finish in the money after Week 13, we will be sending a W9 form with a self-addressed stamped envelope for you to fill out in the next week or so. But if you want to fill out a form beforehand you can do so by clicking on the link below:

Simply fill out the form and fax it back to my attention (this is very important) at 715 445-4087.

Hi Tom,

I have not received my check yet for D/C Tampa league. You should have my info from last years check. Any problems please let me know.
